Register as New Patient
The doctors welcome new patients who live within our practice areas.
All new patients complete a health history questionnaire and are offered a health check if they would like one.
Medical treatment is available from the date of registration. You will be asked to provide photo identification or two forms of written identification to confirm your address when registering. Please contact reception for further information.
The Practice Catchment Area
For patients living in the Holsworthy and Hatherleigh area please ring 01409 253692 to be advised. Stratton patients call 01288 352133 to be advised. Please have your postcode handy!
Pre-registration With the Practice
If you wish to pre-register click on the link below to open the forms. When you have completed all of the details, click on the "Submit" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct. You will also be asked for photo identification or two forms of written identification to confirm your address.
As some addresses fall within both the Stratton and Holsworthy catchment areas please specify which surgery you would like to be registered at within one of the information fields.
Please note - until we have seen your ID and the form has been signed you will not be registered onto our system.
Registering for on-line access
Once you have been registered with a practice, you can apply for on-line access. If you wish to register for on-line access to your patient record, order repeat medications, book appointments and check on test results etc. please click HERE

Temporary Patient Registrations
If you are ill whilst away from home, or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Disabled Patient Facilities
Our surgery is accessible to patients using a wheelchair. We also have three parking spaces outside our practice which are reserved for patients displaying a disabled sticker.
For patients with hearing problems, we have a hearing loop in the reception and main waiting room. There is also a portable hearing loop available from reception. Please remember to adjust your hearing aid accordingly.
For any other needs, you or your carer should request help at our main reception desk where we will try to assist you.
