All patients at the practice have a named GP. Your GP is responsible for the coordination of your health care and services. When you register at the practice you will be told which GP's list you have been put on and you will be asked to see this GP when attending the medical centre. If you do not know which GPs list you are on then please ask at reception or ring in.

Nurse Practitioners

Teresa North-Lewis

Teresa has a  BSc Hons in Special Community Healthcare Nursing. She has been a full-time nurse since 1978 and has nursing experience in A&E, triage, cancer care, ear, nose and throat conditions, eye care, paediatrics and geriatrics as well as general and district nursing. As a Nurse Practitioner Teresa is part of the ON THE DAY TEAM and has been prescribing since 2003.

Sarah Westley-Thomas

Sarah trained at University Hospital in London and became a community nurse in 1989. She spent many years as a District Nursing Sister and Practice Nurse and qualified with BSc (Hons) as a Nurse Practitioner in 2001. She has special interests in respiratory medicine including asthma and COPD.

Bridget Chisholm

Bridget joined the practice in November 2015.

Jane Wason

Jane joined the practice in November 2016. Prior to this she was Community Matron serving the Holsworthy area.

Carol Ball

Carol is an Advanced Nurse Practitioner. She joined the practice in August 2018 and, once our Hatherleigh base is ready, will be based at Hatherleigh.

Julie Leatherbarrow

Julie started with us in 2019 and is based at our Stratton site.

Judy Stevens

Judy is currently based at our Holsworthy practice, having newly qualified as a nurse practitioner. She will be moving back to Stratton after a 3 month placement.


Our highly qualified practice nurses deal with a range of conditions and health concerns. They are experts in many areas of disease management. Their individual specialties are indicated below.
Claire Gough

RGN, BSc Hons in Community Nursing, Nurse Prescribers, Diploma in Adult Nursing, Masters in Parkinson’s disease management. Claire is our Senior Nurse and is a part of the management team at the practice.

Hannah Daniel

RGN Dip Prof Studies in Nursing. HOLSWORTHY. Hannah provides general medical care. She has worked as a practice nurse since 1999 prior to which she worked in general surgery at NDDH. She is involved in running the Coronary Heart Disease clinic. She also has an interest in leg ulcer and wound management.

Kathryn Shadrick

RGN. HOLSWORTHY. Kathryn provides general medical care and care for patients who suffer with asthma.

Jenny Ley

RGN. HOLSWORTHY Jenny provides general medical care, weight loss advice and annual health checks.

Carol Clarke

Carol joined the practice in September 2015 and works at both Holsworthy and Stratton sites.

Debbie is based at Stratton.

Ann Stevens

Ann is based at Stratton.

Joanne Crocker

Joanne is based at our Stratton surgery.

Healthcare Assistants

Our nursing team is supported by Health Care Assistants. They can take blood, check blood pressure, test urine and undertake simple dressing and removal of sutures.
